How to use it
  1. Select your tax filing status.
  2. Enter your MAGI from 2 years ago.
  3. Choose whether to include an estimated Part D IRMAA surcharge.
  4. Read your total monthly premium and IRMAA breakdown.
Formula
Your income tier is determined by comparing your MAGI from two years prior against six CMS-published brackets. Each bracket sets a fixed total Part B premium and, optionally, a fixed Part D surcharge. The IRMAA surcharge shown is your tier's total Part B premium minus the standard $202.90 premium.
Worked example

A single filer with $250,000 in MAGI two years ago falls in the $205,001-$499,999 IRMAA tier, paying $649.20/month for Part B β€” a $446.30 surcharge over the $202.90 standard premium.

Interpreting your result

IRMAA is assessed on your MAGI from two years earlier β€” 2026 premiums use your 2024 tax return. If your income has since dropped due to a qualifying life event, you can request a redetermination using SSA Form SSA-44. Married filing separately uses a much lower, different income threshold not modeled here.

Recommendations
  • β€’ IRMAA is a cliff, not a smooth ramp β€” a dollar of extra income that pushes you over a bracket line can add tens of dollars a month.
  • β€’ Brackets shift most years.
  • β€’ If your income has since dropped due to a qualifying life event, SSA Form SSA-44 lets you request a new determination.
Frequently asked questions
The Social Security Administration sets each year's IRMAA brackets using the most recent tax return data available to it, typically from two years prior.
